What Does a Medical Science Liaison Do?

A Medical Science Liaison (MSL) is a field-based scientific expert employed by a pharmaceutical, biotech, or medical device company. Unlike sales representatives, MSLs do not promote products — they engage in peer-level scientific exchange with healthcare professionals, researchers, and key opinion leaders (KOLs) about the evidence behind a therapy or technology.

If you imagine having the kind of clinical conversations you used to have at grand rounds — discussing trial design, mechanism of action, real-world outcomes — but now as a full-time job with a company car and no weekend call, that is a reasonable approximation of the MSL role.

MSLs manage a geographic territory, setting their own schedules to visit academic medical centers, community practices, and medical conferences. You are the company's scientific resource in the field, gathering insights, answering evidence-based questions, and supporting clinical trial site relationships.

How to Transition from Clinical Practice

  • Target therapeutic areas where you have specialty depth — companies want real expertise
  • Build a LinkedIn presence highlighting your publications, presentations, and KOL connections
  • Apply with a business-style resume, not a CV — hiring managers are not clinicians
  • Reach out to current MSLs in your specialty for informational interviews
  • Consider a contract MSL role to get your first year of field experience
Most MSL hiring managers say the biggest differentiator is scientific credibility in the therapeutic area, not prior industry experience.

Common Questions

Do I need a PhD to become an MSL?
No. Many MSLs have MD, DO, or PharmD degrees. A PhD is common in some oncology and rare disease roles, but clinical experience is often more valued than a research doctorate.
Is the MSL role like a sales job?
No. MSLs are not measured on sales metrics. They are scientific resources for healthcare professionals. The work is medical affairs, not commercial.
What does a territory look like?
Usually a multi-state region. You manage your own schedule, planning visits to academic medical centers, community practices, and conference appearances.
How competitive is MSL hiring?
Very. These roles are popular and most companies want relevant therapeutic area expertise plus some industry exposure. Entry-level positions exist but require intentional preparation.
